A case manager may visit you during your hospital stay. The case manager will work with you and your family to coordinate any services you might need during your hospitalization and after discharge. Whenever possible, please arrange to have family or friends available to transport you to your home before noon. If transportation by family or friends is not possible, let your case manager know. The case manager can help you make other arrangements.
