Advance Directive & POLST Workshop - No Cost

FREE class available to anyone in the community that would like more information on Advance Directives and POLST. The class explains the difference between a POLST and Advance Health Care Directive, lists the responsibilities of a health care agent and discusses the advantages of creating a POLST or Advance Directive. Assistance will be available for those wishing to create directives after class. 3 Continuing Education Units (contact hours) may be available.
