Congestive Heart Failure Telephone Treatment

The CHATT program helps you manage congestive heart failure (CHF). Whether you've recently been diagnosed with CHF or struggled with it for years, we can help.

Communication and Education

Our program can help you improve your quality of life, reduce CHF complications and help keep you out of the hospital. This service includes frequent telephone calls from a registered nurse who specializes in cardiovascular care. We also help patients understand heart failure, and what they can do to control symptoms. Our program will show you how to:

  • Understand your heart & cardiovascular system
  • Increase your self-confidence
  • Reduce the risk factors that could lead to more heart problems
  • Take advantage of the latest cardiac nutrition guidelines
  • Understand your medications
  • Control your symptoms
  • Know when you need to seek medical attention

Teamwork can improve your health

The CHATT program uses a multi-disciplinary approach, which includes your physician, the CHATT nurse, CHATT medical director, dietitians, pharmacists, medical social workers, laboratory technologists, cardiac rehabilitation nurses, home caregivers, your family, and most importantly, YOU!

Getting started is easy:

  • Obtain a referral to CHATT from your physician
  • Complete a physical evaluation and interview with the CHATT nurse
  • Desire to better manage your heart disease
